A Patient Review Instrument (PRI) is an assessment tool designed by New York State Department of Health to assess the medical, physical and cognitive characteristics of prospective nursing home residents as well as to document selected services that they may receive. This assessment gathers specific information to include: patient demographics, medical conditions and treatments, selected behaviors, specialized services and diagnoses of current medical conditions. This information is used to paint a picture of the patients current medical, physical and cognitive status.
All New York State skilled nursing facilities require a PRI and Screen to enable them to determine a patient's condition for long-term placement. This assessment is also used to determine Medicare and Medicaid reimbursements.
A Screen is an assessment used to determine the patient's ability to return to the community or qualify for long-term placement. Both of these assessments are to be completed prior to the admission to any long-term care facility.
New York State mandates that specially trained Registered Nurses may complete a PRI; and the Screen may be completed by a specially trained Registered Nurse or Social Worker.
Trained individuals have been certified through the New York State Department of Health.
Assisted living facilities must have proper pre-admission assessments completed for prospective residents of their facilities as well. These assessments are completed by Registered Nurses in accordance to their scope of practice and facility policy.
Hi my name is Stephanie Smith and I am a highly experienced healthcare professional with over 25 years of experience in the field. I hold a Bachelor of Science in Nursing (BSN) and have worked in a variety of healthcare settings, including hospitals, assisted living, and long-term care facilities.
One of the things that I am most passionate about is working with the senior population. For the past 15 years, I have dedicated my career to providing compassionate and comprehensive care to older adults. I believe that every individual, regardless of age or health status, deserves to be treated with dignity and respect.
In addition to my BSN, I have obtained certification in both the Patient Review Instrument (PRI) and Screen and nurse educator. These certifications have allowed me to provide specialized care to individuals with complex medical needs and have given me the skills and knowledge to coordinate care across multiple disciplines.
Currently, I am working towards my Master of Science in Nursing Leadership and Management. I believe that strong leadership is essential to providing high-quality healthcare, and I am committed to developing the skills and knowledge necessary to become a successful leader in the field.

The start to my career was as a young teenager working in the kitchen as a dietary assistant at a local nursing home. I loved working in the nursing home mostly because I gained relationships with many of the residents there. They would look forward to seeing me for their evening meal and loved listening to my stories about my high school activities. Being in this environment and learning to love the interaction with these precious elderly individuals inspired me to start a career in the nursing field. I signed up for the certified nursing assistant course that was provided at this facility. Unfortunately, I had to wait until I turned 18 years old. Once I turned 18, I quickly completed the training and became an eager new Certified Nursing Assistant. The knowledge I learned in the roll was invaluable and inspired me to continue my education towards a Registered Nurse.
As a registered nurse my career extended through, acute care, assisted living, long term care, and home care settings. I have gained a deep understanding of the unique challenges and needs of patients in each of these environments, and I have worked hard to provide compassionate, high-quality care to all of my patients.
One of my areas of expertise is in working with senior populations. I approach each patient with empathy and a commitment to delivering the best possible care.
I am also certified in PRI&SCREEN, which allows me to perform patient assessments as directed by the department of health. This certification has been an invaluable tool in my work with patients across different settings, allowing me to provide tailored care that is both effective and compassionate.
In addition to my technical skills and certifications, I believe that my ability to connect with patients on a personal level is what truly sets me apart as a nurse. I strive to build trust and rapport with each patient and their families, and I always take the time to listen to their concerns and answer any questions they may have.
